You can complete our intake form two ways, DOWNLOAD or ONLINE, see below for more detailed information.


DOWNLOAD option:

  1. Download the Intake Form

  2. Complete, Print and Sign

  3. Submit via website, fax, mail or in-person (Oahu)

 

ONLINE option:

  • EACH member of the buying unit will need to complete a form.

  • Every buying unit must have a PRIMARY CLIENT form.

  • If there are more members in your buying-unit (i.e.: members included in your purchase or will participate in the classes, coaching sessions), they must complete the CO-CLIENT form. Please indicate the relationship and name of the Primary Client on the Co-Client intake form.

  • We use a third party, RightSignature for secure submission of our online forms.

    • After you click on the link to get started, you must complete the screen requesting your name and email address before the intake form appears.

    • After submitting the form online, you will need to click a verification link via your email account you entered prior to completing the intake form. Please be sure your email address is correct when you enter your email address in the first popup box. RightSignature will send a verification email to that email address. If there is typo, your intake form is incomplete and you must to redo your form.

  • HHOC will email you an acknowledgement of your intake form and payment received within 3 business days with your next steps.
